Procedures for Requesting Personal Information Disclosure

Privacy Policy

If you wish to request a disclosure of personal information, please take the appropriate procedures based on the following.

I. Scope of Disclosure

The scope of personal information disclosure is limited to the applicant's address, name, the purposes of information usage by Ascot Corp. (hereafter, "the Company"), and any other personal information that has been acquired and stored by the Company.

III. Required Documents

  • A. Request application form specified by the Company
    Print out the Personal Information Inquiry Application Formand fill in all required items.
  • B. Personal identification documentation

    1.If the applicant is personally requesting disclosure of their own personal information, submit a photocopy of one (1) of the following identification documents:
    a. Driver's license
    b. Passport
    c. Health insurance card
    d. Residence card
    Please note that the identification document submitted must display the same name and address as those entered on the Company- specified application form.

    2. If a proxy is requesting disclosure on behalf of the applicant, submit both items a and b below:
    a. A letter of authorization specifying the proxy
    b. A photocopy of the seal certification document for the applicant’s seal on the letter of authorization, or a personal identification document for the proxy (one [1] of the documents stipulated in III-B-1)

    3.If a relative or adult guardian of the applicant is requesting disclosure on behalf of the applicant, submit both items a and b below:
    a. One (1) of the following: family register (official copy), family register abstract, certification from a family court, certificate of registered matters, or any other document that certifies one's relationship with the applicant.
    However, said document's date of creation must be no more than 30 days prior to the personal information disclosure request application date. The section containing legal domicile information must be blacked out.

    b. A photocopy of the personal identification document for the relative or adult guardian making the application (one [1] of the documents stipulated in III-B-1).

IV. Processing Fees

  • ARequest for notification or disclosure of purpose of usage
    - Processing fee: ¥1,000 (incl. tax) per request
    - Payment method: Please enclose a fixed-amount postal money order certificate (teigaku kogawase shosho) for the above amount together with the application documents you are submitting

  • B.Requests other than those specified in IV-A
    - Processing fee: Not applicable.
    However, if it is not possible to specify the personal information for which the request is made, you must first request disclosure of the personal information held by the Company. In this case, please note that the fee described in IV-A above for disclosure requests will be required.

V. Response Method and Time

  • A.Response method
    The Company will contact the applicant via the desired response method as specified in the application form.

  • B. Response time
    The time required for a response will be approximately two (2) weeks from the date of receipt of all documents, etc. as specified in section III above. Please be aware that delays may occur due to reasons beyond our control, such as postal service operating conditions and/or the nature of information requested.

VI. Cases in Which Disclosure is Not Possible

The Company is unable to carry out information disclosure in the following cases. Please note that the fee paid will not be refunded even if the disclosure is not made.

  • A.The identity of the applicant cannot be verified or the eligibility of the proxy cannot be verified

  • B.When the type of personal information requested is not subject to disclosure, etc.

  • C.The application form specified by the Company has not been completed correctly or in full

  • D.The processing fee has not been paid in full

  • E.Fulfilling the disclosure request may harm the life, body, property, or other rights or interests of the applicant and/or any third party

  • F.Revealing the existence or nonexistence of the relevant personal data may cause harm to the life, body, or property of the applicant and/or any third party

  • G.Revealing the existence or nonexistence of the relevant personal data may encourage or induce illegal or unjust acts

  • H.Revealing the existence or nonexistence of the relevant personal data may harm national security, damage relations of trust with other countries or international organizations, or cause a disadvantage in negotiations with other countries or international organizations

  • I.Revealing the existence or nonexistence of the relevant personal data may interfere with the prevention, suppression, or investigation of crimes, or with the maintenance of other public safety and order

  • J.Fulfilling the disclosure request may cause significant hindrance to the smooth operation of the Company's business

  • K.Fulfilling the request is or may be contrary to any laws or regulations

  • L.Other cases in which the Company deems disclosure, etc., to be inappropriate

VII. Purpose of Usage for Personal Information Obtained through Procedure for Requesting Disclosure

Personal information obtained by the Company through the Procedure for Requesting Disclosure will be used only to the extent necessary for the investigation for the Procedure, identification of the application or his/her proxy, collection of the fee, and response to the Procedure for Disclosure, etc.
All documents submitted will be disposed of in an appropriate manner after the response to the request for disclosure is completed.
